CREATING A REGISTRATION FORM
Before students can register for practicum credits, they must submit a Field Confirmation Form (FCF) to indicate they have obtained an eligible internship. The Denver-based program already had a university-built system for this process, but the two satellite programs did not. I was charged with developing an alternate means of obtaining the same information using survey software. I designed the completed-survey confirmation to match the Denver FCF to maintain consistency of forms across programs. When it came to the interface, however, I took the opportunity to build a quality user experience through visual cohesion and economy, precision writing, and "if-then" logic.

Branding
I had a rare opportunity with this project to create a visually appealing and well-branded landing page. In addition, I utilized many of Qualtric's customizable features to create a branding "feel" throughout the survey, including the progress, selected buttons, the headers for each section, and emphasize text. Whenever I used color as a signifier I also bolded or italicized the font to be sensitive to users that may not perceive the colors.
As always, simplicity and clarity were my key objectives in designing the page. I limited large blocks of text and eliminated excessive use of bold, italics, and underlines for emphasis.




Improving on the Standard
My objective with this project was to create a Field Confirmation Form (FCF) that matched the already extant FCF for the Denver program. However, I knew there were key aspects that I wanted to tweak to improve usability and to avoid some of the confusion cause by the old form.
For instance, students frequently submitted FCF's for the wrong number of credits. Rather than allowing students to enter the number of credits, I redesigned the FCF to allow them to select standard credits or non-standard credits. If they chose non-standard, "if-then" logic would display a prompt to meet with their program director to complete a Field Credit Excepetions form.
Mobile Friendly
Thanks to Qualtrics, my design was converted to a mobile friendly form. Nonetheless, I had to pay close attention to both mobile and desktop versions to make sure that my text and visuals worked in both circumstances.
